 Hi,
I am Perfusionist from india, we have used cutodial for 10 patients, our results were good so far. we give at a rate of 150ml/mt/m2 for 8 mts either through pump or through the quick priming line(especially for adults) directly through the pressure bag at a temperature of 2-4degrees.our arrest interval was extented upto 65 minutes. So far it is working well, and we are satisfied using it especially for sick myocardium and cases expected to have prolonged cross clamp intervals.

We are currently using the HTK solution as a primary cardioplegia at UPMC Presbyterian University Hospital in Pittsburgh, PA. I am doing some research into this as a cardioplegia agent as opposed to its intended use as an organ preservative. We typically give 2L initial dose; normally no further delivery is required. Blood gas analysis post cardioplegia reveals a serious drop in sodium (usually <126 mmol/L) accompanied by a drop in calcium (<0.96mmol/L).  While the HTK solution is designed to do this for organ preservation, I am curious as to its effects when introduced systemically through cardioplegia delivery (usually antegrade).  Any data concerning the use of HTK as a cardioplegia solution would be appreciated.

In my thirty years doing pediatrics, HTK, delNido & Rebeyka have been the best formulas especially with single dosing.  Multidosing simply pumps more air into the myocardial tissues.  For many years we would rest the heart and tap the air trapped in the coronaries.  Single dosing is equal or better if one uses them.  My preference is for HTK with ischemic times over 200 minutes.  There also are some very long times with delNido but not so long with Rebeyka.
